Please be aware that New Leaf Nutrition Counseling may call your insurance company to assist you in verifying coverage as a courtesy but we cannot guarantee payment from your insurance company. Any charges denied by your insurance company will be billed directly to you. If you have questions or concerns, you may find it helpful to contact your insurance company directly.
Good Faith Estimate
Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges.
Under the law, health care providers would need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for services. You have the right to receive a “Good Faith Estimate” explaining how much your care will cost with our practice so as of January 1, 2022, if you do not want us to file with your insurance, a Good Faith Estimate will be provided by upon scheduling and/or as requested.
Please be aware that if you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
We will save all benefit quotes from your insurer in our EHR but we encourage our patients to save a copy of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises.
****Please note that while most insurance companies do not require a referral, medical records are preferred as they can assist our providers in providing adequate care*********
